When you schedule your first appointment for a therapy session, you are required to fill out and read forms regarding personal information, insurance/payment information and HIPPA regulations.

You can print out the required forms, fill them out at home or work and bring them with you to your first appointment. This should save you time at your first appointment and make the process flow more smoothly.
